At Health Fund for Children (HF4C) we know that out-of-pocket costs for healthcare can be completely out of reach for some families. We exist to help children in need get the quality healthcare they deserve regardless of race, religion, gender or citizenship status. Families with children are assessed for need, on a case-by-case basis. HF4C will financially assist children 18 years and younger who have no insurance, underinsured and whose parents make less than 250% of the Federal Poverty Guidelines.

HOW APPLYING WORKS

After you submit an application we will contact you to discuss your case and talk to you about our eligibility requirements for assistance. 

OUR CURRENT ELIGIBILITY REQUIREMENTS INCLUDE:

  1. Child’s Personal ID: ID, passport, Birth certificate
  2. Proof of Residency: Photo ID with address lease, in parent’s name
  3. No medical insurance and no eligibility for Medicare or Medicaid
  4. Proof of parent’s income: copy of paycheck or letter from employer, tax return
